Short answer: Upheal is better value for most full-caseload therapists: $39 a month for unlimited AI notes, a genuinely useful free plan, and an optional EHR. Mentalyc is cheaper for part-time or light caseloads (from $14.99 a month for 40 notes on annual billing) and has deeper clinical features on its higher plans, like automatic CPT codes, supervision notes and group notes. Both include a BAA for every account and say they don't train AI on your sessions.

Mentalyc prices by note volume (monthly / annual per month):

  • Mini: $19.99 / $14.99 for 40 notes
  • Basic: $39.99 / $29.99 for 100 notes. Adds treatment plans and "Alliance Genie" alliance scores.
  • Pro: $69.99 / $59.99 for 160 notes. Adds couples, child and family clients, EMDR, play and psychiatry notes, supervision notes and automatic CPT codes.
  • Super: $119.99 / $99.99 for 330 notes. Adds group therapy notes.

There's a 14-day free trial with full Pro access and 15 notes, plus a discount for pre-licensed clinicians (Mentalyc pricing).

Upheal charges a flat $39 a month for unlimited AI notes from video, audio, dictation and uploads, including couples and families, treatment plans and built-in telehealth. The Free plan covers dictated and uploaded notes for individual therapy. AI Assistant Pro (+$39) removes the monthly cap on its chat assistant. As a full EHR, Upheal costs $1 a session or $99 a month flat. There's a 30-day money-back guarantee and 50% off the first year for students and associates (Upheal pricing).

The break-even: at up to about 100 notes a month, Mentalyc Basic ($29.99 annual) costs less than Upheal ($39). At a full caseload of 25+ sessions a week (about 100+ notes a month), Upheal's unlimited plan is cheaper than Mentalyc Pro ($59.99) or Super ($99.99), unless you need Mentalyc's CPT codes, supervision or group notes.

Privacy and compliance

Both say the right things, but read the details:

  • BAA: Upheal's BAA takes effect when you click "Accept Upheal Business Associate Agreement" or accept electronically. It says Upheal "will use only de-identified, aggregated, anonymous information to improve the application" (Upheal BAA). Mentalyc says "every account is covered by our BAA," created at signup and downloadable from your profile (Mentalyc security).
  • Audio: Upheal deletes session audio by default once notes are ready, though you can choose to keep it. Mentalyc says audio is deleted once the note is ready, and keeps the anonymized transcript until you delete it. Neither page gives an exact retention window in hours, so confirm it in writing if your policies require one.
  • AI training: Mentalyc states plainly that it doesn't use session data to train AI models. Upheal's BAA limits improvement work to de-identified, aggregated data.

Whichever you choose, you'll still need client consent. Several states now require written consent for AI scribes (state law guide; free consent form template).
